Players assume the role of noble families with properties in the countryside around Messina. They try to rescue people from the plague by moving them to their estates.

As the plague in Messina worsens, players will build small independent communities, while trying to control the plague by fire. In the final rounds of the game, the plague begins to subside and players return rescued citizens to repopulate the resilient city of Messina.
